Key Steps for Completing Form 941 When Wages Were Paid
- Download the Form & Instructions:
Get the latest version from the IRS website. Use one form per quarter. - Business Information:
Enter your EIN, legal business name, trade name (if any), and address on both pages. - Quarter Selection:
Check Q4 (October–December). Do not combine multiple quarters on one form. - Payroll Data:
- Line 1: Number of employees paid during the quarter.
- Line 2: Total wages for Q4 only (not cumulative for the year).
- Line 3: Federal income tax withheld.
- Lines 5a & 5c: Social Security and Medicare wages.
- Calculate Social Security tax (12.4%) and Medicare tax (2.9%) for both employer and employee portions.
- Thresholds:
- Stop Social Security tax after annual wages exceed $176,100.
- Additional Medicare tax applies after $200,000 per employee.
- Totals & Adjustments:
- Line 6: Total tax before adjustments.
- Line 12: Total tax after adjustments and credits.
- Line 13: Deposits made during the quarter.
- Line 14 or 15: Balance due or refund.
- Depositor Status:
- If total tax liability is under $50,000 for the last four quarters, you are a monthly depositor.
- Report monthly liabilities in Part 2.
- Final Steps:
- Sign and date the form.
- Mail to the IRS using the address for your state (without payment if paid online).
- Use USPS certified mail for proof of filing.
- Q4 return is due by January 31.
Special Case: If No Wages Were Paid in Q4
- Line 1: Enter 0 employees.
- Line 2: Enter 0.00 wages.
- Check the box for “No wages subject to Social Security and Medicare.”
- Line 12: Enter 0.00 (do not leave blank).
- Leave other lines blank unless instructions require zeros.
- Check the box in Part 2 for liability under $2,500.
- Sign, date, and mail using the “without payment” address for your state.
- Filing online is also an option.
