Get Form 941 and instructions from the IRS website. Use one form per quarter. - Business Information:
Enter EIN, legal business name, trade name (if any), and address on both pages. - Quarter Selection:
Check Q4 (October–December). Do not combine quarters on one form. - Key Entries for Zero Wages:
- Line 1: Enter 0 employees.
- Line 2: Enter 0.00 wages.
- Line 12: Enter 0.00 (do not leave blank).
- Leave other lines blank unless instructions require zeros.
- Check the box for “No wages subject to Social Security and Medicare.”
- Depositor Status:
If Line 12 is less than $2,500, check the first box in Part 2. - Other Sections:
- Part 3: Leave blank unless business closed or seasonal employer (read instructions before checking).
- Part 4: Indicate if IRS should contact you or someone else.
- Part 5: Sign, date, and include your title and phone number.
- Voucher:
Leave the payment voucher section blank because no tax is owed. - Mailing Instructions:
Use the “without payment” address for your state (e.g., Iowa).
Mail via USPS certified mail for proof, or file online. - Deadline:
Q4 return is due by January 31 for the quarter ending December 31. - Tip:
Keep copies of the filed form and mailing proof for your records.
