Line 1: Number of employees that you paid wages during the quarter you are filing Form 941.
Line 2: Enter the total wages for the quarter
Line 3: Enter the total Federal Income Tax Withheld from employees’ wages.
Line 5: Enter taxable part of social security total wages and Medicare wages in column 1.
Column 2 of Line 5 is the total of social security and Medicare (half from employees’ wages and the other half from you the employer)
Taxable wages includes sick pay, fringe benefits that are taxable
Line 5e calculates the total social security tax and Medicare tax you have to collect and pay (employee and employer sides included)
